Sadler Clinic Participates in the First Annual Relay For Life Conroe Event

Over 1.5 million new cancer cases are expected to be diagnosed this year.  Sadler Clinic is joining the American Cancer Society by teaming up to help the fight this deadly disease.  Sadler Clinic, its team of physicians and employees, will participate in the Relay for Life of Conroe on April 29th to 30th. Sadler Clinic participated in the Relay for Life of The Woodlands on April 8th and 9th and rose over $10,500.  As a sponsor of both events, Sadler Clinic is currently raising funds and encouraging patients, physicians, employees, friends and family members to join their team to walk in honor of those who have fought this disease with such bravery.  “The event begins with a survivors’ lap, where hundreds of cancer survivors and their caregivers take a victory lap around the track. We welcome all area survivors and caregivers to join us for this moving and symbolic event that helps celebrate the progress that has been made in improving cancer survival rates and the quality of life following cancer treatment,” said Cathy Pierantozzi, Director of Human Resources and Marketing at Sadler Clinic.  Cathy is a three-year breast cancer survivor and is serving as Survivorship Chair for Relay for Life of Conroe.  “Later in the evening, we hold a Luminaria Ceremony.  This touching event takes place after sundown, honoring the community’s cancer survivors and remembering those lost to this terrible disease.  We dim the lights and circle the track surrounded with glowing luminaria that bear the name of someone who has battled cancer; it’s a very moving experience.”

Dr. Thomas Stark – ENT and Ryan Putnam – Clinical Supervisor have committed to shave their heads should Sadler Clinic reach its goals of 75 participants and $10,000 in fundraising.  Dr. Stark came up with the idea for this fundraiser.  Relay is a cause that is close to Dr. Thomas Stark’s heart, as he lost his wife to Leukemia in 2003.  “Along the way she commented to me something that I think lots of cancer fighters notice.  Even with a scarf or cap other people look at you differently. Most gazes are filled with love and compassion, but a few of us in the cancer-free world try to avoid eye contact or stay away as if cancer is contagious. Either reaction is just a reminder that things are not the same in the world of a human being under attack by cancer.” By shaving his head, Dr. Stark hopes to show support for the battle those affected by cancer fight and encourage others along the way to give back.

The Relay for Life event represents hope that those lost to cancer will never be forgotten, that those who face cancer will be supported, and that one day cancer will be eliminated.

Congratulations to our Sadler "SuperDocs"

Congratulations to Dr. George Boyle, Dr. Carrol Cagle, and Dr. Randall Johnson for being selected to the Texas SuperDocs 2010.  Super Doctors are selected in a thorough process that combines peer nomination, peer evaluation by practice area, and independent research and examination.  Thousands of ballots are sent out to doctors asking them to nominate colleagues they know to be exceptional in their field of practice.  The nominees are then evaluated on a set of established criteria including: years of experience, hospital appointments, fellowships, professional activities, leadership positions, academic achievements, board certifications, publications, honors and awards, and other outstanding achievements. The nominees are then evaluated by a "blue ribbon panel."  The Super Doctors list is limited to 5% of the physicians within their respective state or region.  Congratulations again to our "SuperDocs!"

Sleep specialist warns of inappropriate use of sleeping pills

Sleep specialist warns of inappropriate use of sleeping pills
By Victoria Wright

Often when someone has difficulty sleeping, a pill becomes the ticket to slumber. When a person complains of not sleeping well too often, the doctor prescribes a sleeping aid instead of getting to the root of the problem.

Many will self-medicate with an over-the-counter sleep aid. The "sleeping pill" industry is estimated to gross more than $2 billion a year, according to the research firm IMS Health.

Taking that nightly pill out of desperation can have dire results, such as drug dependency, rebound insomnia, addiction and problems with withdrawal. Often, the medication can hide the underlying problem. Rather than sedating your brain into a sleep, there are well-established methods now developed and used by trained sleep specialists. An internationally recognized expert, Jerald H. Simmons, M.D., neurologist and sleep disorders specialist and director of the Sadler Clinic Sleep Disorders Center, takes a detailed approach to solving the problem of insomnia without simply relying on sedative hypnotic medication.

According to Simmons, by obtaining a detailed history and examination, specific treatments can be administered that are tailored for each patient.

Insomnia can be divided into two large categories: Sleep initiation insomnia is when a person has difficulty falling asleep; or sleep maintenance insomnia is when someone has difficulty staying asleep.

"Sleep maintenance insomnia is very common, and the main key to successfully treat it is to identify the cause for the awakenings. These patients complain of repeatedly awakening from their sleep. Commonly, these awakenings are caused by breathing problems that relate to snoring and obstructive sleep apnea (OSA), which occurs while asleep when the throat muscles relax." Simmons siad. "Typically, those affected are unaware of the breathing disturbance because it resolves when the muscles tighten-up on awakening. The awakening becomes a protection of the airway, so to speak. Taking a sleeping pill can worsen the breathing problem by preventing these awakenings, and some medications even enhance the relaxation of the throat muscles, which worsens the breathing problem. This happens all too frequently and can lead to cardiac disease and hypertension as well as other medical problems.

"When a person has difficulty staying asleep, it is important to determine the cause for the awakenings in order to properly treat the problem. That is where a sleep study, if done properly, can provide a proper diagnosis. So often, I have seen patients troubled by repeated awakenings who are prescribed sleeping pills; and when they come to my center, we find that they suffer from OSA, which is a serious breathing problem during sleep. Doctors are making a terrible mistake by simply prescribing sleeping pills in patients who repeatedly awaken from sleep rather than ensure the patients have a proper sleep evaluation."

Other common causes for awakenings include: restless legs syndrome, depression, periodic leg movements and pain.

"When sleeping pills help a patient with undiagnosed OSA sleep better, it creates a problem because they think they are properly treated since they are staying asleep," Simmons said. "The concern is that the ongoing OSA causes cardiovascular disease and other medical problems over time."

The second type of insomnia, sleep initiation insomnia, can be from a variety of causes, ranging from poor sleep habits to psychological factors. It's clear that proper behavioral changes are the key to recovery in many of these patients. Simmons said treatment for this type of insomnia includes: Changing lifestyle, working on stress reduction and learning effective methods to wind down and relax before bed. Sleeping pills have a proper role as well.

"It is frequently important to use a combination approach consisting of improvements in sleep hygiene or sleeping habits and proper use of specific medications that do not have addictive properties or lingering effects during the day," Simmons said. "Some medications that became available over the past 15 years are much safer than older medications. Also, most over-the-counter sleep aids have antihistamines in them, which cause drowsiness. There are many side effects that can occur with these types of medications as well, and their easy accessibility results in inappropriate use by many."

If you or someone you know takes increasingly higher dosages of sleeping pills with declining benefits, then a big problem is bound to develop over time and proper assistance should be considered. For more information, contact the Sadler Clinic Sleep Disorders Center at 281-297-6305 or go to Dr. Simmons' website, www.HoustonSleep.net, for more information.

For more than three decades there has been a growing recognition that some people have difficulty focusing and maintaining attention, in various instances they may also have difficulty sitting peacefully in a classroom or calm setting. These individuals have been labeled with Attention Deficit Disorder-with or without Hyperactivity-known as ADD/ ADHD.

Traditional treatment for ADD/ADHD has been with stimulant medications, such as Ritalin or Adderall, which provides improvement by stimulating a fatigued, unrested, brain. It is now known that problems with sleep can result in difficulties with concentration and the ability to stay focused during the day. The increased physical activity-hyperactivity-displayed by many of those affected is a way of remaining stimulated thus overcoming the sleepiness. This difficulty is most noted when sitting inactive and under-stimulated.

About 30% of these patients have a sleep disturbance as a fundamental cause, according to Jerald H. Simmons, MD, an internationally recognized Neurologist and Sleep Disorders specialist. Simmons is director of the Sadler Clinic Sleep Disorders Center and has been amongst the first who have understood the association between ADDD / ADHD and sleep disturbances. He has treated these types of patients for about 20 years.

If a sleep disorder is present, there are other treatment options for improvement of the ADD / ADHD symptoms. " If there is a concern that a child or adult has ADD / ADHD then it is imperative to first make a few observations regarding the person's sleep in order to determine if a sleep disorders center consultation is necessary, prior to starting stimulant drugs." Simmons states, "In many cases those who have been placed on stimulants can be successfully taken off them if a sleep problem has been properly diagnosed and treated. For years now I have taken this approach and many patients labeled with ADD /ADHD in whom we have diagnosed with a sleep disturbance have demonstrated drastic improvement after treating their sleep disorder. This treatment has minimized or eliminated their reliance on stimulant medications."

The main causes for the disturbed sleep in the ADD / ADHD population is from: A lack of enough sleep because of conditions in the home stemming from domestic / environmental factors; Restless Legs Syndrome or Obstructive Sleep Apnea or an associated condition known as the Upper Airway Resistance Syndrome (UARS), according to Simmons. UARS is a subtle breathing disorder not properly diagnosed at most sleep centers but because Simmons's centers utilize extra measures his team is able to recognize and treat this disorder properly. He adds, "We look at a whole battery of issues when someone complains of ADD/ADHD, including psychological and physiological factors. I've seen many patients improve when we treat their sleep without the need for stimulants."

An example of one such patient is Trey Girlinghouse a nine-year-old whose teachers became concerned with his difficulties in school. They encouraged his mother to find medical assistance for what they labeled as ADD. He was placed on stimulant medication treatment; however his mother was concerned about medication usage and looked for alternative treatments. She had Trey evaluated by Simmons and after a comprehensive sleep test he was found to have UARS. He then began treatment for this breathing disorder and is no longer on stimulants and he has markedly improved his performance at school. Angela Girlinghouse-his mother-states," Now that his sleep is better and he does better at school, the teachers agree that he no longer needs to be on medications."

Simmons provides a partial list of signs that should raise concern when considering if ADD / ADHD may be from a disturbance with sleep. These are:

  • Difficulty falling asleep

  • Difficulty staying asleep

  • Snoring or labored breathing

  • Kicking repetitively during the night

  • Excessively grinding or clenching teeth

  • Difficulty awakening in the morning

  • Sleep walking or sleep talking

  • Noticeable sleepiness during the day when not active.

Simmons concludes, "If someone has any of these indicators and appears to suffer with ADD / ADHD symptoms then a proper sleep evaluation would be warranted." Patients can be evaluated by contacting the Sadler Clinic Sleep Disorders Center at 281-297-6305 or visiting www.HoustonSleep.net for more information and where they can fill out a questionnaire to initiate an evaluation.

Snoring linked to increased risk of stroke

Written by: Victoria Wright, Sadler Clinic

What sounds like simple snoring could lead to major health issues. Snoring is associated with a larger problem called Obstructive Sleep Apnea (OSA) both of which are now known to increase the risk of stroke. May is National Stroke Awareness Month: As part of this initiative, by the National Stroke Association, the Sadler Clinic Sleep Disorders Center is reaching out to our community to heighten awareness of this seemingly benign nighttime nuisance that increases the risk of stroke.

Anyone can have a stroke regardless of age, race or gender. Understanding the risk factors and controlling them by proper treatment can prevent the devastation of strokes. In our community, we are fortunate to have a world-class sleep disorders center that can help those with snoring and OSA. Jerald H. Simmons, MD a Triple Board-Certified Neurologist who directs the Sadler Clinic Sleep Disorders Center states, "It's amazing how many people have an increased risk of stroke from snoring and OSA who go undiagnosed. Subsequently they suffer a stroke or heart attack that could have been prevented. Recent studies are amazing; for example, it has now been shown that loud snoring alone, over many years, can cause narrowing of the carotid arteries in the neck. The nightly vibration of snoring leads to disruption of the blood vessel wall that initiates arterial sclerosis to develop, narrowing of blood vessels thus leading to stroke. There are other factors too, such as extreme fluctuations in blood pressures while asleep from OSA, that creates damage to the blood vessels of the brain."

According to studies reported by the American Sleep Disorders Association:

  • Loud snorers have a 67 percent greater likelihood of having a stroke, compared with people who do not snore.

  • Loud snoring is associated with an increased use of health care resources (emergency visits and hospitalization), when compared to non-snorers.

  • Even quite snoring can be a concern and has been show to be associated with an increased risk for hypertension in women.

Recently, Conroe Regional Medical Center (CRMC) became a local stroke center to serve our community. The program, directed by Neurologist, Randall Wright, MD who also works at the Sadler Clinic, has implemented protocols at CRMC that utilize treatment methods that help stroke patients when they develop their sudden symptoms. If you think you may be suffering from a stroke, do not wait, call 911 for immediate care.  To learn more about stroke and get specific details on this devastating condition visit www.stroke.org.

Interestingly, women are twice as likely to die from stroke than breast cancer, according to Healthy Women, the nation's leading independent health information source for women.   The survey also found that 425,000 women suffer from stroke each year in the USA; 55,000 more than men. Some of these women may have a small jaw, a large tongue, or large tonsils, all factors that can lead to a narrow airway causing snoring and OSA. Simmons states "Following menopause, women develop changes in fat tissue deposition in their airway resulting in increased snoring and occurrence of OSA. We have become extremely successful in treating patients of all ages and both sexes for OSA and snoring by utilizing a variety of techniques which we tailor for the individual patient. Our success rates are much higher than the national average."

Missing link between teeth grinding and sleep is discovered by Sadler Clinic doctor

CLICK HERE TO VIEW THE ARTICLE IN THE CONROE COURIER!

Written by Victoria Wright, Sadler Clinic, vwright@sadler.com

Teeth clenching or grinding-known as bruxism-is a common problem that may lead to headaches, facial pain, and TMJ Disorder. Bruxism has puzzled dentists and physicians for years therefore limiting treatment success of these disorders.

Research conducted by neurologist and sleep disorder specialist, Jerald H. Simmons, MD, of the Sadler Clinic Sleep Disorders Center has demonstrated that a main cause for bruxism relates to breathing problems during sleep such as snoring and obstructive sleep apnea (OSA). With years of observation and experience in treating patients with OSA and bruxism, he recognized the relationship of these conditions. This led him to seek out the assistance of Ron Prehn, DDS, where together they studied over 700 hundred patients with OSA. This is the first research in the world to ever conclude that bruxism, while asleep, is an attempt to bring the jaw and tongue forward. Bruxism stops the back of the tongue from blocking the airway and is the brain's way of preventing this blockage from occurring.

Simmons treats patients with a variety of methods ranging from

  • masks that fit over the nose to deliver positive pressure,

  • special dental appliances that bring the jaw forward to open the back of the airway, and

  • surgery to remove crowded tissue in the back of the throat.

He also finds it helpful in some patients to use a combination of all these treatment approaches. He frequently seeks the assistance of Prehn to make customized continuous positive airway pressure (CPAP) masks for some patients. These are held on the face using a special dental appliance, thus eliminating the need for straps on the head and chin.

"I saw multiple doctors and none of them were able to take care of my symptoms. As soon as Dr. Simmons' treatment opened up my airway my clenching stopped," said patient Diane Fitch. "I think Dr. Simmons' research has found that missing link."

Other conditions that the Sadler Clinic Sleep Disorders Center has excelled in treating include

  • ADHD, which is now recognized to frequently be a result of poor, non-restorative sleep;

  • fibromyalgia and chronic fatigue, both also now known to be a result of problems with sleep; and

  • morning headaches, a condition usually resulting from disturbances in sleep that is not properly diagnosed in many patients.

Simmons and Prehn have presented their results to both dentists and physicians at national medical and dental conferences. This new concept has created a tide within the dental field because most dentists are challenged by patients who have bruxism. Now they have a new prospective and alternative approach in treating these patients.

"It is amazing how much improvement we can provide in patients by properly treating disturbances in their sleep," said Simmons. "We are fortunate in this community that the Sadler Clinic has provided me the opportunity to build a sleep center that surpasses the quality of those in large academic institutions."

Simmons' sleep center at the Sadler Clinic is the largest in Montgomery County and amongst the largest in the Houston area. For more information on his work and the sleep center, go to www.HoustonSleep.net where you can watch interviews of patients with TMJ and other conditions he treats.

SAME DAY APPOINTMENTS AT SADLER CLINIC

Navigating the complicated world of medical care no longer needs to be hopelessly inconvenient, confusing and thoroughly disenchanting due to long wait times. Not too long ago, long waits and delays in the office were seen as something of a status symbol. A physician who had a long wait list for appointments or a long delay in the waiting room was perceived as an awfully good physician to be in such a "demand." The one thing patients had in common was that no one was happy, with much of the dissatisfaction centered on this issue of long waits and delays. Sure, you can plan your annual physical months in advance, but what if you wake up one morning with a sore throat? Or what if that hacking cough you've had seems to have become bronchitis? For either ailment, you're probably not sick enough to go to an emergency room, but you're not sure you'll be able to get a same-day appointment with your regular physician. The 2009 Survey of Physician Appointment Wait Time, reports average doctor appointment wait times in five different medical specialties. Leading their list was Boston, which has patients waiting an average of 49.6 days to get a new-patient appointment. On average, Houston's wait time was 23.4 days, placing it at number 4 among 15 large metropolitan areas (source: Houston Business Journal).  One of the causes of the high cost of health care is unnecessary trips to the emergency room when you can't make an office appointment.

Many Sadler Clinic physicians are currently offering same day appointments. With the many health care options available, it is comforting to know that Sadler Clinic has made it a priority to offer patients both convenience and accessibility.  Please contact our offices today and begin your journey back to health.

TRADITIONAL MEDICARE STILL ACCEPTED

Sadler Clinic recognizes that changes in healthcare coverage for Medicare eligible participants have presented numerous uncertainties to patients when choosing a medical plan.  Historically, traditional Medicare was the only option available for those eligible for coverage.   Now there are many options to choose from.  Medicare Advantage plans are available from private companies that contract with the Centers for Medicaid and Medicare Services (CMS), the federal agency that administers the traditional Medicare program.

Traditional Medicare often allows the patient more flexibility when making physician and hospital choices, not restricting the patient to a limited network of healthcare providers.

Per Rob Branstetter, Chief Executive Officer for Sadler Clinic, "Montgomery County currently has over 41,000 Medicare-eligible participants and of those, 78% choose to stay in traditional Medicare.  If a Sadler patient chooses traditional Medicare as their health insurance program, they can have confidence in knowing that Sadler Clinic will continue to be their home for healthcare services.  At Sadler Clinic, we respect our patients' right to choose, and welcome the opportunity to serve traditional Medicare patients in the area."

Sadler Clinic, the largest multispecialty clinic in Montgomery County, offers healthcare in over 30 different specialties, as well as comprehensive diagnostic capabilities including their new Radiation Therapy Center in The Woodlands.  The Clinic has served the needs of the citizens of Montgomery County and surrounding areas for over 50 years.  They currently have eleven locations in the area: Conroe, The Woodlands, Willis, and Montgomery.  Their newest location at League Line Road in Conroe is scheduled to open in late summer 2009.

SADLER CLINIC OPENS NEW FACILITY AT 9305 PINECROFT DRIVE IN THE WOODLANDS

Sadler Clinic opened is newest location at 9305 Pinecroft Drive in The Woodlands in November 2008. The 80,000 sq. ft. facility will provide a variety of services that are new to Sadler Clinic. Sadler’s Urgent Care Center is currently open M‐F from 9AM – 6PM. Directed by Dr. Kent Erickson the Urgent Care Center is available to take walk‐in patients that need to be seen for routine immunizations to minor emergencies and occupational services. The Sadler Clinic Radiation Therapy Center is Montgomery County’s first Radiosurgery and Image Guided Radiation Therapy Accelerator. The Center will showcase Elekta Synergy © Linear Accelerator technology with the world’s most advanced Cone Beam Image Guided Radiation Technology, Intensity Modulated Radiation Therapy and non‐invasive precise Stereoknife Radiosurgery. Stereoknife Radiosurgery can provide an important alternative to potentially dangerous anesthesia and surgery for patients with cancer and unacceptable cardiovascular risks for the operating room. The new facility will also house Sadler Clinic’s first Ambulatory Surgery Center on the second floor of the 9305 Pinecroft location. Sadler physicians will be able to perform outpatient procedures in the center. The ASC encompasses 20,000 square feet. It will have four operating rooms and four procedure rooms. We anticipate the center to be fully functional by March 2009. In addition to these new centers, Sadler is proud to employ one of the fastest commercially available PET/CT scanners allowing us to offer precise image accuracy combined with astonishing digital image quality. This unique image modality combines two types of medical imaging on a single piece of equipment. The CT images aid the physician in evaluating the anatomic structure of the body, while the PET images allow the physician to evaluate the bodies’ functions. When the two image types are combined the physician gets a complete depiction of the patient’s health. The physicians and staff welcomed distinguished members of The South Montgomery County Woodlands Chamber of Commerce for a Business After Hours reception on November 11th, which coincided with the Sadler’s 50 year anniversary. Mayor Garry Watts of Shenandoah presented Sadler with a proclamation during their City Council Meeting on Wednesday, November 12th and Mayor Webb Melder of Conroe presented a proclamation to Sadler Clinic on Thursday, November 13th. Both commended the physicians of Sadler Clinic and thanked them for providing the community with outstanding healthcare these past 50 years. The clinic’s official open house, which included a special appearance by Santa Claus, was held on December 6th. As Mr. Branstetter, Executive Director of Sadler Clinic stated “This latest facility is a testament to our founder, Dr. Deane Sadler’s commitment to families and their health and we look forward to continuing Dr. Sadler’s legacy for years to come”.
